Encore Rehabilitation, a leading rehabilitation provider, is seeking a Physical Therapist to work part-time, 20-30 hours/week, in our skilled nursing facility, Greystone Healthcare Center, located in Blountville, TN / Tennessee. New Grad Physical Therapists are welcome to apply! This is a trach/vent facility with patients with high acuity. The work is tough, but the team is great, and makes it fun and it’s very rewarding.

ENCORE! Rehabilitation Services partners with skilled nursing facilities to provide exceptional physical, occupational and speech therapy services.

Requirements for the Physical Therapist position:

  • Part-time
  • Tennessee license required
  • Physical Therapy degree required

Physical Therapist / RPT / PT

Functions under physician’s orders. Provides and directs physical therapy services to residents, including assessment, treatment, program planning and implementation, and completes related documentation and communication.
Common programs include: ambulation training, strengthening and conditioning, contracture management, wound care, cardiac rehab, balance training, modalities, orthotics, and back injury protection.


  1. Bachelor of Science in Physical Therapy from an accredited program.
  2. Successful completion of National Certification Examination for RPT.
  3. Current state licensure or license eligible.
  4. Regular attendance.
